چکیده

E LEVATION of arterial pressure has been found in patients during attacks of angina pectoris1-3 and relief of exertional angina has been reported in hypertensive patients following satisfactory control of hypertension.4' 5 Further, anginal pain has been found to occur in hypertensive patients during lowering of arterial pressure.4 Although these observations suggest an etiologic correlation between angina pectoris and level of arterial pressure, this has not been adequately established. The present study was undertaken to determine the possible relationship between arterial pressure and exertional angina pectoris in hypertensive patients with and without coronary arterial disease. Methods Seven hypertensive patients (four women and three men) in the Research Ward of the Cleveland Clinic Hospital with unequivocal history of exertional angina pectoris of the functional class II were studied. Coronary arteriography by Sones' technic ;6 revealed segmental disease of the left coronary artery in four and of both coronary arteries in one (fig. 1); the remaining two were found to have normal coronary arteries (fig. 2). Radiographic, as well as electrocardiographic, evidence of left ventricular hypertrophy was present in all. None had a history or electrocardiographic evidence of recent or remote myocardial infarction. The five patients with coronary arterial disease had not taken any antihypertensive treatment prior to the study and during the study had moderate to severe hypertension; the two patients with normal coronary arteries had received guanethidine

Effect of exercise on electrocardiograms of patients with low serum potassium.

DURING a recent study of the relationship between arterial pressure and exertional angina pectoris in hypertensive patients, it was observed that one patient who had hypokalemia but a normal control electrocardiogram exhibited electrocardiographic signs typical of potassium deficit following exercise. Because of this incidental observation, further investigation was carried out to evaluate the ...

Acute arterial hypertension during spontaneous angina in patients with fixed coronary stenosis and exertional angina: an associated rather than a triggering phenomenon.

The role of systemic arterial hypertension as a possible trigger of myocardial ischemia during angina at rest was studied in 13 consecutive patients who also had a history of exertional angina. Significant (greater than or equal to 70%) stenosis of at least one major vessel was present in each of the 10 patients in whom coronary arteriography was carried out. After documentation of the electroc...

Investigation of the physiological basis for increased exercise threshold for angina pectoris after physical conditioning.

Eight patients with coronary heart disease and exertional angina pectoris successfully completed an 11-15 wk program of endurance exercise conditioning. Angina threshold was determined by upright bicycle ergometer exercise and by atrial pacing. The product of heart rate and arterial systolic blood pressure at the exercise angina threshold was higher after conditioning. suggesting that condition...

Comparison between Revascularization and Optimal Medical Therapy in Patients with Stable Angina Pectoris

Background and Purpose: Regarding usefulness of revascularization versus optimal medical therapy in patients with stable angina pectoris, data are challenging. The aim of this 12-month follow-up study was to compare the survival benefit associated with revascularization versus optimal medical therapy on the patients with stable angina pectoris. Materials and Methods: A prospective clinical s...
